During this appointment, Dr. Mindy Worthington sits with you and asks about how you have been feeling — mentally, emotionally, and physically. She asks about your past, your current life, your sleep, your stress, your relationships, and any symptoms that have been bothering you.
This is not a test you pass or fail. There are no right or wrong answers. It is simply a conversation — a deep, careful one — so that Dr. Worthington can understand what you are going through and figure out the best way to help you.
By the end of the evaluation, you will have a clearer picture of what is affecting your mental health and what your next steps should be.

Write down your symptoms Before you come in, take a few minutes to note how you have been feeling — how long, how often, and how much it affects your daily life. It does not need to be detailed. Even a few bullet points help.
Bring a list of your medications Include any prescription drugs, vitamins, supplements, or over-the-counter medications you take regularly.
Bring past records if you have them If you have seen a psychiatrist, therapist, or doctor before and have old records or diagnoses, bring them. Dr. Worthington will review them — but do not worry if you do not have them.
